How To Apply For Services

Early Start

Early Start Referral Form (Ages 0-3 years old)

Lanterman Referral

Lanterman Developmental Services Referral Form (Ages 3 and older)

Early Start Referrals (online form)
Infants and toddlers under the age of 3 are referred to VMRC by doctors, preschools, hospitals, family members, child care centers/providers, etc.  This is considered to be an Early Start referral because the child is under 3 years of age.  The child will be referred directly to an Early Start Intake Coordinator who begins the eligibility determination process. It is important to meet legally-mandated timelines so that the child and family can begin receiving services as soon as possible.

Lanterman Developmental Services Referrals (online form)
People over age 3 are referred to VMRC by schools, hospitals, doctors, other families, etc.  When they or their families contact VMRC, persons three years of age or older are assigned an Intake Coordinator whose job is to acquaint the applicant and his/her family with VMRC and its services, coordinate the diagnostic and eligibility determination process.

Who to Contact for Services:
To refer a child or adult for an assessment of eligibility:

In San Joaquin County call our Stockton Office: (209) 955-3281 for under age 3 (Early Start Services) and (209) 955-3209 for over age 3 (Lanterman Act Services)

In Stanislaus County call our Modesto Office: (209) 557-5619 for under age 3 (Early Start Services) and (209) 557-2197 for over age 3 (Lanterman Act Services)

In Amador, Calaveras and Tuolumne County call our San Andreas Office: (209) 674-6535 for both under age 3 (Early Start Services) AND over age 3 (Lanterman Act Services)

Under 3 Years Old (Early Start Services)
Click here to make a referral for Early Start.

Over 3 Years Old (Lanterman Act Services)
An interdisciplinary team consisting of the Intake Coordinator, various specialists, and the family, determines whether the applicant has a substantial handicap caused by a developmental disability. This process requires written evaluations and various diagnostic tests (usually at least a medical, psychological or developmental evaluation).

i know a family daughter is about 10 or 11. She got evaluated for speech services in school and her eye sight is poor.
Family doesn’t have medical and barely make living. I went last weekend and they were out of food. Mother also has some trauma in life. Mother was premature but in her country she never received any services. when she migrated to usa she didnt have medical till this day to receive services. She never made it to high school. They have older daughter too but due to medical issues they didnt get evaluated. If you know anything that might help them please let me know. Mother has immigration status issues. Father petitioned mother, she had green card but initially mothers family were on asylum after marriage mother was sponsored by father which got approved and hse had green card which expired lin august. Mother appeared for citizenship interview but she was questioned on trauma she had in her country. Mother doesnt want to speak about it. So answered anyhow. Mother went to anxiety and depression. She passed citizenship test but immigration officer asked questions on asylum which brought back trauma and shock to mother. Immigration office confused mother’s case with mother’s sisters case. thinking that mother has asylum approved. Immigration attorney sent application to renew green card but its still pending. Sp 60 percent of income is going to attorney fees.
Looking at their situation with no medical and limited food supplies, its worse than mine.
Please let me know if you can guide them

My son, who is 2, was diagnosed with asd last October. Since then I have been attempting to get an referral for an evaluation for myself as I believe I fell through the cracks in the 80s and 90s when women simply weren’t thought to be able to be autistic by most clinical specialists. However, MRS wouldn’t give me a referral….idk how to go about doing this. Can I self refer?

My son was refer to get evaluated (Developmental & Behavioral Pediatrics) but they said the next available appointment will be in 8 months. What the wait at your facility? and do I need to get another referral? Do you take Cigna insurance? My son is five years old and the closest office will be in Modesto.

    1. Doug Bonnet says:

Hi Paola, I just reached out to you via email to ask for a bit more information. My apologies for the late reply as I was out on medical leave.

  61. Hi, I’ve called before and was told that it was unlikely that I would be able to be seen as I wasn’t diagnosed until recently, despite ASD signs and symptoms from before I was 18, I’m 30 now, in San Joaquin. I called again today, diagnosis in hand, and was told the same thing. My diagnosing Doctor says y’all can’t do that because of the Lanterman Act.
I’ve never gotten a call back when I’ve left messages before I had my diagnosis, and was told I didn’t qualify when I had MediCal insurance in 2018. I have private insurance now with a PPO, so hopefully I’ll actually get a call back or an email.

    1. Doug Bonnet says:

Hello. You can contact our Clinical Intake Manager, Lynda Macabales, at 209-955-3377, or via email at lmacabales@vmrc.net and she will be able to assist you.
