Valley Mountain Regional Center (VMRC) and the Department of Developmental Services (DDS) track serious incidents involving the people we serve. This helps us recognize patterns and take action to keep our clients safe and healthy.
Under Title 17, Section 54327, vendors and long-term care facilities are required to report certain incidents, including suspected abuse or neglect, serious injuries or accidents that need medical attention, unplanned hospitalizations, and missing persons.
Even if a consumer is not physically with the vendor at the time, any death or situation where the consumer becomes a victim of a crime must still be reported. VMRC also asks for reports of incidents involving restraints, emergency room visits, law enforcement or protective services involvement, natural disasters, or negative media attention.
Submitting Special Incident Reports (SIRs)
SIRs can be sent via email or fax to your designated office listed below. For transportation-related SIRs, please contact our transportation broker, R & D Transportation Services, Inc., at (209) 362-2199.

The Coalition of Local Agency Service Providers (CLASP) is an association of Valley Mountain Regional Center (VMRC) service providers. CLASP serves as the professional advisory committee for the regional center, representing the interests and perspectives of service providers on key committees, including the Consumer Services Committee, Nominating Committee, and Finance Committee, and holding a seat on the VMRC Board of Directors.
CLASP meetings are held monthly via video conference from 10:00 a.m. to 12:00 p.m. on the 4th Monday of every month.
Membership in CLASP supports the coalition’s efforts to advocate for service providers and strengthens collaboration with VMRC. Annual membership dues are $35.
