FY 27-28 Quality Incentive Program

Provider Directory Validation Required for FY 2027–2028 Quality Incentive Program Participation

All current vendors must review and validate their Provider Directory record by September 30, 2026. This is required to meet the 2027–2028 Quality Incentive Program (QIP) full rate model.

The Department of Developmental Services (DDS) uses information listed in the Provider Directory to send official communications, updates, and survey invitations. Accurate provider information helps ensure that vendors receive timely notices related to QIP requirements, deadlines, and related program activities.

Action Required

All current vendors are asked to review their Provider Directory record and confirm that the information on file is complete, accurate, and current. This includes contact information, administrative details, and any other relevant provider information listed in the directory.

Information for New Service Providers

New service providers will receive the initial QIP Provider Survey after vendorization has been completed and the Provider Directory record has been validated. Once the survey is issued, new providers must complete it within 60 days.
